Job summary

myPlace Health in Los Angeles, CA is hiring a Social Worker (MSW) for its PACE program. You will plan, organize, and deliver psychosocial services tailored to each participant’s needs, maintaining a caseload and collaborating with the IDT for seamless, person-centered care.

You will support transitions, community resource connections, and Medi-Cal processes while engaging families and caregivers to promote independent living at home.

Qualifications

• MSW from an accredited university.
• CPR and First Aid certification required (or within 30 days of hire).
• 2 years of experience conducting psychosocial assessments, care planning, and case management.
• 2 years of experience with frail or elderly populations.
• Current California Driver’s License with clean driving record and personal auto insurance.
• Proficiency in EMR/EHR documentation preferred.
• Experience collaborating with multidisciplinary or medical teams preferred.
• Bilingual abilities in English and Spanish, Cantonese, Mandarin, or Vietnamese are a plus.
• Ability to lift/move items up to 25 pounds as needed.
• Schedule Monday–Friday 8am–5pm onsite at our center in Los Angeles, CA 90057

Responsibilities

• Engage in daily IDT meetings to ensure participants’ voices are heard, conduct psychosocial assessments and develop care plans.
• Present assessments at care plan conferences for collaborative IDT-developed plans.
• Collaborate with the IDT to guide smooth care transitions between hospitals, homes and facilities.
• Connect participants, families, and caregivers with community resources and provide ongoing support.
• Facilitate transitions between home and other placements while ensuring safety and empowerment.
• Support participants with Medi‑Cal applications and coordinate with government agencies.
• Coordinate advanced care planning with providers in line with goals of care.
• Design and lead group counseling sessions to promote emotional well‑being and social engagement.
• Document care plans and interactions accurately in the EMR for smooth coordination.
• Provide discharge planning support when participants leave the program.
• Embrace flexibility and teamwork; take on other duties as assigned.
• Support SCAN’s Vision and Goals in daily operations.
• Other duties as assigned.
