Job summary

Carle Health is seeking an experienced pharmacist to deliver comprehensive pharmaceutical care to oncology patients within the Carle Cancer Center. You will support specialty care across hematology/oncology, Urology, Ophthalmology, Gynecology, and research, with active involvement in trials and interdisciplinary rounds.

The role emphasizes direct patient care, MTM, dosing optimization, regimen monitoring and policy development to enhance safety and outcomes in an inpatient/outpatient setting.

Qualifications

• Licensed Pharmacist - Illinois IDFPR.
• Bachelor’s Degree in Pharmacy (Required).
• Clinical work experience is preferred.

Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration and monitoring.
• Medication Therapy Management (MTM): Provide MTM services and patient education.
• Dosing Recommendations: Determine optimal doses based on age, weight and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams (inpatient and outpatient).
• Prepare investigational and standard chemotherapy medications for trials.
• Attend rounds and provide services in interdisciplinary practice at Carle Foundation Hospital.
• Collaborate to enhance patient-centered care.
• Monitor and manage adverse effects; ensure adherence and manage toxicities.
• Oversee chemotherapy preparation; support transitions of care and trial protocols.
• Develop medication policies, procedures and safety initiatives.
• Participate in pharmacokinetics dosing service activities and hospital committees.
