Qualifications

• Licensed Pharmacist with IDFPR licensure
• Bachelor’s Degree in Pharmacy (Required)
• Clinical work experience

Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring within the facility.
• MTM: Provide MTM services and patient education as defined by policy.
• Dosing: Determine optimal doses based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams.
• Prepare investigational and standard-of-care medications for clinical trials.
• Attend rounds and provide pharmacy services in interdisciplinary care.
• Monitor adverse effects and adherence for regimens.
• Oversee chemotherapy preparation and transitions of care.
