Qualifications

• Licensed Pharmacist certification (IDFPR).
• Bachelor’s Degree: Pharmacy required.
• Clinical work experience required (oncology preferred).

Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring practices.
• Medication Therapy Management (MTM): Provide MTM services and patient education per policy.
• Dosing Recommendations: Determine optimal doses based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams; inpatient and outpatient care.
• Support clinical trials: prepare investigational and standard medications.
• Participate in rounds; provide pharmacotherapy expertise to the care team.
