Responsibilities

Direct Patient Care: Intervene to improve prescribing and monitoring within the facility.
Medication Therapy Management (MTM): Provide MTM services and patient education.
Dosing Recommendations: Determine optimal doses based on age, weight, and organ function.
Collaborate with hematology/oncology and inpatient pharmacy teams (inpatient and outpatient).
Prepare investigational and standard of care medications for clinical trials.
Attend hematology/oncology rounds and provide pharmacy services on the interdisciplinary team.
Support transitions of care and precept students and residents.
Develop medication-related policies and safety initiatives.
