Job summary

Qualifications

• Doctor of Pharmacy (PharmD) is required or equivalent.
• Illinois licensure as a Pharmacist preferred.
• Strong knowledge of medication safety and regulatory compliance.

Responsibilities

• Identify, prevent and resolve medication-related problems.
• Provide authoritative information to health care professionals and patients.
• Ensure integrated pharmacy services for patient-centered care across the department.
• Maintain documentation in EMR and ensure compliance with policies.
