Qualifications

• Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy Degree is required.
• Active Clinical Pharmacist license in the state of Illinois (IDFPR) is required.
• Preferred: residency or hospital pharmacy experience in specialty areas.

Responsibilities

• Interprets medication orders to ensure safety and proper use, coordinating solutions with pharmacy, nursing and prescribers.
• Monitors therapy for interactions and adjusts therapy per clinical results.
• Maintains comprehensive patient profiles and documentation for all clinical activities and interventions.
• Educates patients and families about medications and answers questions from healthcare professionals.
