Qualifications

• Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy Degree.
• Active Clinical Pharmacist license in the state of Illinois (IDFPR).

• Completion of a pharmacy residency program or equivalent hospital pharmacy experience.

Responsibilities

• Interprets medication orders to ensure safety and proper use, coordinating with pharmacy, nursing and prescribers.
• Monitors therapy for significant drug interactions and intervenes when needed.
• Communicates potential problems and solutions to the prescriber.
• Develops and presents orientation and education programs for staff and learners.
