Qualifications

Responsibilities

• Interprets medication orders to ensure safety and proper use, coordinating with pharmacy, nursing and prescribers.
• Monitors therapy for significant drug interactions and therapeutic duplications; intervenes as needed.
• Documents clinical activities and interventions in the EHR and maintains patient profiles.
• Educates patients and families about specific medications and potential adverse effects.
• Responds to emergencies to assist with medication management and patient care.
