• Preferred: completion of a pharmacy residency or equivalent hospital pharmacy experience; training in specialty areas (neonatology, critical care, oncology).

Responsibilities

• Interprets medication orders to ensure safety and proper use of medications; coordinates solving medication-related problems among pharmacy, nursing, and prescribers.
• Monitors medication therapy for significant drug interactions and intervenes as needed.
• Maintains comprehensive pharmacy patient profiles and documents clinical activities and interventions.
