Qualifications

• Monitors medication therapy for significant interactions and intervenes as needed.
• Evaluates and documents appropriate medication choices to achieve therapeutic outcomes; communicates issues and solutions to prescribers.
• Oversees medication dispensing processes and verifies orders before patient care delivery; maintains patient profiles and formulary compliance.
