Job summary

Northwestern Medicine is seeking a Clinical Pharmacist to ensure safe and effective medication use across our hospital system. You will interpret orders, monitor interactions, and coordinate with physicians, nurses, and pharmacists to optimize patient care.

The role requires a Doctor of Pharmacy or equivalent and an active Illinois Pharmacist license. Preference is given to candidates with residency or specialty training and hospital experience.

Qualifications

• Pharmacist degree (PharmD or equivalent) is required.
• Active Illinois Pharmacist license (IDFPR) is required.
• Preferred: residency or specialized training (neonatology, critical care, oncology).

Responsibilities

• Interprets medication orders to ensure safety and proper use, coordinating with pharmacy, nursing and prescribers.
• Monitors therapy for drug interactions and intervenes as needed.
• Evaluates medications for appropriateness and communicates issues to prescribers.
• Acts as consultant on pharmacokinetics and adverse reactions to healthcare professionals.
• Processes medication orders into the EHR and maintains patient profiles.
• Oversees packaging, labeling and distribution of medications and maintains inventories.
• Compounds and dispenses pharmaceuticals including sterile products and parenteral nutrition.
• Educates staff and patients about medications and participates in training programs.

Education

Bachelor's Degree in Pharmacy or Doctor of Pharmacy
