Job summary

Boston Senior Home Care seeks a Resident Services Coordinator to support residents in aging in place at Supportive Housing sites. You will coordinate services, arrange transportation and translation, and connect tenants with home care and other supports.

Based in designated buildings, you will work with building staff to identify needs, maintain resident files, and ensure timely delivery of programs. This full-time, non-exempt role requires collaboration and strong organizational skills.

Qualifications

• BS or BA degree required in sociology, social work, gerontology or related field.
• Six months or more years of elder care case management experience required.

Responsibilities

• Serve as a resource person for the assigned Supportive Housing building and promote self-sufficiency.
• Develop and maintain relationships with on-site partners (building manager, assistant manager, maintenance).
• Disseminate information to residents by creating monthly calendars and newsletters in multiple languages.
• Maintain a Supportive Housing office and ensure confidentiality of information.
• Collect, report and analyze data for program improvement and funder requirements.
• Plan outreach to identify residents in crisis and coordinate community referrals.
• Direct on- and off-site social activities, workshops, and wellness programs.
• Document resident interactions and maintain files according to BSCH standards.
