Responsibilities

• Review residents' needs and link to community programs and services.
• Serve as the Multi-disciplinary Assessment Team Leader and coordinate care.
• Facilitate care conferences with residents, families and participants.
• Plan and coordinate social, educational, and recreational programs.
• Prepare and distribute a newsletter to keep residents informed.
