Job summary

Old Colony Elder Services (OCES) in Brockton, MA is expanding its housing team and seeking two dedicated professionals to support residents on-site. The roles focus on connecting residents with community programs and coordinating multidisciplinary care to ensure comprehensive service delivery.

You will plan social, educational, and recreational activities, lead care conferences with residents and families, and help produce a resident newsletter.

Qualifications

• Passion for helping others and making a difference.
• Adaptability to changing consumer/program needs.
• At least two years in human services, elder population preferred.
• Bachelor's degree in Social Work or related field preferred.
• Bilingual a plus; Haitian Creole or Cape Verde Creole preferred for one location.

Responsibilities

• Review residents' needs to link to community programs and services.
• Serve as Multi-disciplinary Assessment Team Leader for service coordination.
• Facilitate care conferences with residents, family, and participants.
• Plan and coordinate social, educational, and recreational programs for residents.
• Prepare and distribute a newsletter to inform residents of planned activities.
