Job summary

Boston Senior Home Care is seeking a Per Diem Supportive Housing Tenant Resource Counselor in our Supportive Housing program. The role is on-site, part-time, and per diem, designed to assist residents to age in place, coordinate services, and link to community resources.

The ideal candidate will have a BS/BA in a related field and experience in elder care case management, with the ability to travel within housing sites as needed.

Qualifications

• BS or BA degree in sociology, social work, gerontology or related field.
• Six months or more years of experience in elder care case management required.

Responsibilities

• Serve residents by connecting them to consumer and government resources.
• Coordinate services with on-site partners (building manager, assistant manager, maintenance).
• Train residents on evidence-based classes and conduct trainings.
• Disseminate information via monthly calendar and multilingual newsletters.
