Job summary

Catholic Charities of Baltimore is seeking a Service Coordinator to support residents in aging in place across affordable housing communities. This role links residents to services, coordinates with management and partners, and helps ensure responsive, compliant care plans.

Responsibilities include developing service plans, coordinating resources, and promoting engagement while maintaining accurate records and HUD guidance adherence. Monday–Friday schedule; no remote work.

Qualifications

• Bachelor’s degree in social work, human services, gerontology, psychology, sociology, or a related field.
• Two years’ experience providing social services or service coordination.
• Knowledge of local services for the elderly and people with disabilities.
• Equivalent combination of education, training, and experience may be considered.

Responsibilities

• Develops personalized service plans that identify residents’ needs and connect them to resources.
• Builds collaborations with community partners to expand access to services.
• Promotes resident engagement, social connection, and self-advocacy.
• Responds promptly to resident needs and safety concerns with appropriate supports.
• Documents daily resident communications and service coordination activities.
• Coordinates with staff to identify residents who may need assistance.
• Assists residents, families, and community members with information and resources.
