Job summary

Boston Medical Center’s Living Well at Home Program seeks a Housing & Community Support Specialist to provide on-site stabilization and housing readiness in a congregate setting in Cambridge. You will build trust, assess needs, and guide patients through housing processes with a patient-centered approach.

Responsibilities include coordinating with housing providers and care teams, developing individualized plans, and ensuring timely documentation.

Qualifications

• Bachelor’s degree in a behavioral health or related field OR two years of relevant work experience OR lived experience of homelessness or behavioral health conditions.
• Experience in housing, healthcare, or social services preferred.
• Fluency in English; Haitian Creole or Spanish preferred.

Responsibilities

• Provides intensive stabilization support on-site in a congregate setting.
• Builds rapport and collaborates with patients to set housing and care goals.
• Guides patients through housing applications and tenancy processes.
• Coordinates with housing providers, care teams, and community resources to support housing readiness.
