Qualifications

• Bachelor's degree preferred.
• Experience working within an interdisciplinary team preferred.
• Strong leadership and group skills.
• Knowledge of Adobe and MS Word, Excel, SharePoint, and other Microsoft 365 platforms; Wellsky a plus.
• Must be comfortable with CORI/SORI and health screening requirements; bilingual a plus.

Responsibilities

• Provide case management to oversee and monitor services and care coordination for older adults in the State Home Care Program, including in-person assessments and safety checks.
• Determine eligibility for the State Home Care Program and/or other community programs for residents; make referrals as necessary.
• Participate in or facilitate care conferences with residents, families, LHA, and professionals to discuss needs and resources.
• Coordinate resident programming, outreach, newsletters, calendars, and informational postings with SCES staff.
• Monitor participation in on-site nutrition programs and coordinate with the Nutrition Department.
• Assist with LHA applications, move-in/out procedures, and compliance with EOEA requirements.
• Maintain MOUs with the LHA and submit required EOEA reports; ensure confidential documentation per EOEA/Agency standards.
• Adhere to HIPAA and other confidentiality requirements; provide coverage in Home Care as needed.
