Job summary

Circadia Health in the Los Angeles area is seeking a Nurse Practitioner to join as an on-site, embedded clinician at a partner facility in West Hills, CA. You will see residents, assess, plan, and coordinate care while integrating Circadia’s AI-based tools into daily workflows.

You’ll train staff, translate AI insights into timely interventions, and document notes the same day. The role requires CA license, 2+ years of NP experience with complex adults, and the ability to travel locally; English

Qualifications

• CA licensed NP; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• Minimum 2+ years NP experience with complex adult populations; able to practice independently.
• Valid CA driver's license; comfortable traveling locally.
• Thrives in a dynamic startup environment.
• English required.

Responsibilities

• Direct patient care: conduct initial and follow-up assessments; adjust plans of care; coordinate with facility providers.
• Tech & bedside integration: train and coach staff; integrate Circadia into clinical workflows.
• Data into action: interpret trend-based insights; prioritize high-risk patients; drive timely interventions.
• Close the loop: complete documentation within the same day.
• High-volume rounding: manage visits at multiple SNFs (30+ day when appropriate).
• Mentor & collaborate: support NP onboarding and partner with clinical ops/product teams.
