Job summary

Circadia Health is hiring a Nurse Practitioner for an on-site, embedded role at partner facilities in West Hills, CA. You will combine direct patient care with real-time data insights and hands-on training to integrate Circadia's AI into daily care.

The position requires CA NP licensure, 2+ years of experience with complex adults, and travel within the local area. Competitive salary and full benefits are offered in a dynamic startup environment.

Qualifications

• CA NP license required and ANCC certification preferred.
• 2+ years NP experience with complex adult populations.
• Eligible for Medicare participation; company bills on your behalf.
• Valid CA driver’s license; travel locally may be required.

Responsibilities

• Direct patient care: assess residents, adjust care, coordinate with facility providers.
• Tech → bedside integration: train staff to use Circadia in clinical workflows.
• Interpret trend-based insights to prioritize high-risk patients and intervene.
• Complete documentation within the same day to ensure accurate records.
• High-volume rounding at designated SNFs (30+ visits/day where applicable).
• Provide feedback to improve workflows and tech integration in clinical settings.
• Mentor and collaborate with NP onboarding and clinical ops/product teams.
