Job summary

Qualifications

• Certified and licensed NP in CA (ANCC or equivalent)
• Eligible for Medicare participation; company bills on your behalf
• 2+ years NP experience with complex adult populations; able to practice independently
• Valid CA driver's license; comfortable traveling locally
• Thrives in a dynamic startup environment
• English required

Responsibilities

• Direct patient care: conduct initial and follow-up assessments on flagged residents; adjust plans of care; coordinate with facility providers
• Tech → bedside integration: train and coach staff; standardize Circadia into clinical workflows
• Data into action: interpret trend-based insights; prioritize high-risk patients; drive timely interventions
• Close the loop: complete documentation/charts on the same day
• High-volume rounding: visits across designated SNFs, focusing on new admits and Circadia alerts
• Quality & product feedback: surface field insights to improve workflows and tech integration
• Mentor & collaborate: support NP onboarding and partner with clinical ops/product teams
