Job summary
Circadia Health is seeking an on-site Nurse Practitioner in Los Altos, CA to integrate AI-driven insights into bedside care. You will conduct assessments, initiate care plans, and coach facility staff to apply data-driven interventions.
This full-time role combines patient care with tech-to-bedside integration, live training, and collaboration with clinical ops/product teams to improve outcomes for seniors.
Qualifications
- CA-licensed NP with ANCC certification.
- 2+ years NP experience with complex adult populations.
- Eligible for Medicare participation; the company bills on your behalf.
- Valid CA driver’s license; able to travel locally.
- Thrives in a dynamic startup environment.
- English proficiency required.
Responsibilities
- Direct patient care: initial and follow-up assessments; plan of care updates; coordinate with providers.
- Tech-to-bedside integration: train staff and standardize Circadia workflows.
- Data into action: interpret insights, prioritize high-risk patients, drive interventions.
- Close the loop: complete documentation on the same day.
- High-volume rounding: visits across SNFs with focus on new admissions.
- Quality & product feedback: surface workflow improvements.
- Mentor & collaborate: onboard NP staff; partner with clinical ops/product teams.
