Job summary

Circadia Health is hiring a full-time on-site Nurse Practitioner to work at a partner facility in Sacramento, CA. You will see patients first, respond to real-time health signals, and coach facility teams to translate data into earlier interventions.

Responsibilities include conducting assessments, coordinating care, training staff, and documenting visits with AI-assisted tools. A CA NP license and startup-ready mindset are essential.

Qualifications

• Certified and licensed Nurse Practitioner in CA.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid CA driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required.

Responsibilities

• Direct patient care: initial and follow-up assessments; initiate/adjust plans of care; coordinate with facility providers.
• Tech → bedside integration: Train and coach staff; standardize Circadia into clinical workflows.
• Data into action: Interpret trend insights; prioritize high-risk patients; drive timely interventions.
• Close the loop: Complete documentation within same day.
• High-volume rounding: 30+ visits/day across SNFs where appropriate.
• Quality & product feedback: Surface field insights to improve workflows and tech integration.
• Mentor & collaborate: Onboard NP, share best practices with clinical ops/product teams.
