Job summary

Circadia Health is seeking an on-site Nurse Practitioner in Los Altos, CA, to lead direct patient care while integrating AI-driven predictions into bedside workflows. You will train facility staff, interpret risk signals, and drive timely interventions to reduce readmissions.

The role combines clinical autonomy with hands-on tech-to-bedside work at a partner facility, Monday–Friday, on-site. English required; Spanish preferred.

Qualifications

• Certified and licensed NP in CA (ANCC or equivalent).
• 2+ years NP experience with complex adult populations; able to practice independently.
• Eligible for Medicare participation; company bills on your behalf.
• Valid CA driver's license; comfortable traveling locally.
• English required; Spanish is a plus.

Responsibilities

• Direct patient care: conduct initial and follow-up assessments; coordinate with facility providers.
• Tech → bedside integration: train staff and standardize Circadia in clinical workflows.
• Data into action: interpret trend insights and drive timely interventions.
• Close the loop: complete documentation on the same day.
• High‑volume rounding: visit 30+ patients/day at SNFs when applicable.
• Quality & product feedback: surface field insights to improve workflows and tech integration.
• Mentor & collaborate: support NP onboarding with clinical ops/product teams.
