Job summary

Circadia Health is seeking a full-time Nurse Practitioner to be embedded at a partner facility in Pasadena, TX. You will conduct patient assessments, integrate Circadia into care, and coach staff to act on AI-driven health signals.

Requirements include TX NP licensure with ANCC, 2+ years of experience with complex adults, and willingness to travel locally. Expect 100% company-paid health benefits, a 401(k), and generous time off.

Qualifications

• TX-licensed NP with ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid CA driver’s license; comfortable traveling locally.

Responsibilities

• Direct patient care: conduct initial and follow-up assessments; adjust plans of care.
• Tech → bedside integration: train staff and standardize Circadia usage in workflows.
• Data into action: interpret trend insights and prioritize high‑risk patients.
• Close the loop: complete notes within the same day.
• High-volume rounding: 30+ visits/day across SNFs, focusing on new admits and alerts.
• Quality & product feedback: surface field insights to improve workflows and integration.
• Mentor & collaborate: support NP onboarding and partner with clinical ops/product teams.
