Health Link is seeking a Scheduling & Intake Coordinator for our South Bay home health operations. This in-person, full-time role supports clinician scheduling and intake processes to ensure timely patient care.

The ideal candidate is organized, proactive, and calm under pressure, with experience in healthcare operations and strong communication with clinicians, patients, and referral sources. Benefits are included.

Qualificações

• One year or more in scheduling, intake, or coordination in healthcare is preferred.
• Strong organizational and time-management abilities.
• Proficient with high-volume, real-time scheduling.
• Clear, professional communication with clinicians and patients.
• Knowledge of medical terminology or healthcare compliance is a plus.

Responsabilidades

• Coordinate and manage daily clinician schedules based on patient needs and staff availability.
• Support intake processes including referral coordination, data entry, and case placement.
• Collaborate with clinical leadership, operations, and business development teams.
• Respond quickly to urgent scheduling changes and patient needs; communicate with clinicians and leadership.
• Assist with timely initiation of care and visit coverage.
• Identify and resolve scheduling or intake barriers proactively.
• Support field staff to ensure patient care is not delayed.
