Job summary

Ennoble Care in Union County, New Jersey is seeking a full-time Care Coordinator embedded at a partnered dialysis clinic to lead on-site patient engagement and care coordination. You will schedule visits, collaborate with providers and facility staff, monitor patient needs, and ensure timely follow-up and accurate documentation while maintaining a compassionate, patient-centered approach.

This role offers weekday hours, no weekends or on-call requirements, and a focus on teamwork and proactive

Qualifications

• Legally authorized to work in the United States of America.
• Active, unrestricted clinical certification or licensure (RN/LPN/MA/CMA/CNA) or comparable credential.
• Dialysis-specific certification (CDT/CHT/CCHT) preferred.
• Experience coordinating appointments in a clinical setting valuable.
• Experience with hemodialysis/nephrology patients is a plus.
• Excellent organizational and time-management skills.

Responsibilities

• Provide on-site engagement with dialysis patients to support care plans and follow-ups.
• Coordinate patient care using Ennoble Care's EMR for continuity and timely follow-up.
• Schedule provider visits and manage daily clinic workflow.
• Collaborate with providers, dialysis staff, and multidisciplinary team.
• Communicate patient needs and condition changes promptly to clinical leadership.
• Maintain accurate documentation in line with policies and regulations.
• Demonstrate professionalism, empathy, and sound judgment in a fast-paced setting.
• Support partnerships with facilities and teams while maintaining technical proficiency.
