Ennoble Care is seeking a full-time Care Coordinator, Embedded to work at a senior living facility in Hamilton/Florence, NJ. You will partner with clinicians, staff, and families to plan and coordinate high-quality care, ensuring clear communication and compassionate service for patients with chronic conditions.

Ideal candidates are organized, empathetic, and skilled at managing multiple care streams. You’ll navigate our EMR system and support families, while maintaining a patient-first approach

Qualifications

• Must speak on the phone for most of the day.
• Compassionate and empathetic toward patients.
• Ability to take accurate notes to document tasks promptly.
• Multitask across patients and workstreams while staying organized.
• Thrive in a fast-paced environment.
• Full-time, Monday–Friday daytime hours in Hamilton/Florence, NJ.
• Proficient in Outlook and Microsoft Office.
• Knowledge of basic healthcare terms and care principles.
• Must pass drug screen and background check; willing to learn new skills.

Responsibilities

• Create individualized patient care plans and manage care using EMR.
• Frequent contact with patients to coordinate care and programs.
• Document all client communications accurately.
• Coordinate with Primary Care Clinician on patient needs and plans.
• Inform Team Supervisor of issues affecting care plans.
• Collaborate with cross-organizational teams to meet patient needs.
• Learn to use secure technology (email, phone, EMR).
• Maintain daily work logs and escalate issues as needed.
• Provide excellent customer service based on patient feedback.
• Support team members to maintain productivity and a positive environment.
