Job summary

Ennoble Care is seeking a Care Coordinator, Embedded @ Partnered-Dialysis Clinics in the Orlando, Florida area. This on-site role engages patients, coordinates care plans with Ennoble providers and clinic staff, schedules provider visits, and ensures timely follow-up.

The position emphasizes accurate documentation, strong organizational skills, and professional communication within a fast-paced clinical environment.

Qualifications

• Legally authorized to work in the United States
• Active, unrestricted clinical certification or licensure required
• Dialysis-specific certification is highly preferred
• Experience coordinating patient appointments in a clinical setting is highly preferred
• Experience with hemodialysis/nephrology patients is highly preferred
• Strong organizational and time-management skills
• Professional communication across multiple platforms

Responsibilities

• Provide on-site engagement with dialysis patients to support care plans, follow-ups, and care transitions
• Coordinate care using the EMR to ensure continuity and timely follow-up
• Schedule patient visits for providers and support daily clinic workflow
• Collaborate with providers, dialysis staff, and care team to support patient-centered care
• Serve as liaison between providers, patients, families, and facility staff with clear communication
• Notify clinical leaders of changes in patient condition or urgent matters
• Identify and resolve barriers to care and documentation
• Maintain accurate documentation per standards and regulations
• Demonstrate professionalism and empathy in a fast-paced setting
• Support teamwork and cross-facility partnerships; utilize Microsoft 365 tools
