Job summary

Ennoble Care seeks a Care Coordinator, Embedded at Partnered-Dialysis Clinics in the Panama City, FL area. This on-site role coordinates patient care, schedules provider visits, and maintains communication with patients, families, and the clinical team.

The ideal candidate will have active clinical licensure, dialysis experience, and strong organizational skills to support continuity of care in a fast-paced setting.

Qualifications

• Legally authorized to work in the United States
• Active, unrestricted clinical certification or licensure such as PCT/CNA/MA/LPN/RN or comparable credential
• Dialysis-specific certification preferred
• Experience coordinating patient appointments in a clinical setting preferred
• Experience with hemodialysis patients preferred
• Strong organizational and communication skills

Responsibilities

• Provide on-site engagement with dialysis patients to support care plans and follow-ups
• Coordinate care using the EMR to ensure continuity and timely follow-up
• Schedule patient visits and support clinic workflow
• Collaborate with clinicians, dialysis staff, and care team
• Communicate patient needs and changes in condition to clinical leaders
• Maintain accurate documentation and ensure regulatory compliance
