Qualifications

• Active, unrestricted clinical certification or licensure (MA/CMA, LPN, RN).
• Minimum 1 year of experience in a clinical/administrative or care coordination role.
• Proficiency with Microsoft 365 (Outlook, Word, Excel).
• Ability to work full-time, Mon-Fri, 8:00am–4:30pm EST in Voorhees, NJ.

Responsibilities

• Coordinate patient-centered care activities using the EMR system.
• Support providers and clinical teams with documentation and follow-up tracking.
• Serve as a key communication liaison between providers and internal teams.
• Communicate patient needs and changes to the clinical team in a timely manner.
• Assist with care transitions, referrals, and coordination with external partners.
• Maintain accurate documentation and support efficient clinical operations.
• Contribute to team collaboration and day-to-day office operations.
