Qualifications

• Legally authorized to work in the United States.
• Active, unrestricted clinical certification/licensure (RN, LPN, CNA, MA/CMA, PCT) or comparable credential.
• Dialysis-specific certification is highly preferred.
• Experience coordinating patient appointments in a clinical setting.
• Proven ability to manage provider schedules and multi-clinic coordination.

Responsibilities

• Engage dialysis patients on-site to support care plans and education.
• Coordinate care using the EMR to ensure continuity and timely follow-up.
• Schedule provider visits and support daily clinic workflow.
• Collaborate with providers and facility staff for integrated care.
• Communicate patient needs and changes promptly to clinical leaders.
• Maintain accurate, timely documentation per standards and regulations.
• Demonstrate professionalism and strong organizational skills in a fast-paced environment.
• Support team collaboration and partnerships across facilities.
