Job summary

Ennoble Care is seeking a full-time Care Coordinator embedded in a Partnered-Dialysis Clinic in the Panama City, Florida area. You will engage patients on-site, coordinate care plans with Ennoble providers and clinic staff, and schedule provider visits to support timely follow-up.

You will monitor patient needs, document changes, address barriers to care, and communicate clearly with patients, families, and the care team using our EMR and Microsoft 365 tools.

Qualifications

• Legally authorized to work in the United States.
• Active, unrestricted clinical certification or licensure (PCT/CNA/MA/LPN/RN or equivalent).
• Dialysis-specific certification is highly preferred.
• Experience coordinating patient appointments in a clinical setting is preferred.
• Minimum 1 year dialysis/hemodialysis patient care experience preferred.
• Excellent organization and communication skills; able to manage multiple priorities.

Responsibilities

• Provide on-site engagement with dialysis patients to support care plans and follow-ups.
• Coordinate care using Ennoble Care’s EMR to ensure continuity and timely follow-up.
• Schedule patient visits for providers and support daily clinic workflow.
• Collaborate with providers, dialysis staff, and multidisciplinary team for patient-centered care.
• Serve as liaison between providers, patients, families, and facility staff with proactive communication.
• Document patient changes and care updates accurately and promptly.
