Job summary

Ennoble Care seeks a Care Coordinator, Embedded at partner dialysis clinics in Oklahoma City. The role engages patients in person, coordinates care plans with Ennoble providers, and ensures timely follow-up and clear communication with patients and families.

The position supports continuity of care, monitors patient needs, and maintains documentation, all under an on-site framework in a fast-paced clinical environment. Strong organizational and communication skills are essential.

Qualifications

• Legally authorized to work in the United States
• Active, unrestricted clinical certification or licensure (PCT/CNA/MA/CMA/LPN/RN)
• Dialysis-specific certification desired (CDT/CHT/CCHT)
• Experience coordinating patient appointments in a clinic
• Experience with hemodialysis/nephrology patients preferred
• Excellent communication and organization in a fast-paced clinical setting
• Ability to work full-time, Monday–Friday, 5:00 a.m.–1:00 p.m. CST, on-site in OKC

Responsibilities

• Provide on-site engagement with dialysis patients to support care plans and follow-ups
• Coordinate care using EMR to ensure continuity and timely follow-up
• Schedule patient visits and support daily clinic workflow
• Collaborate with providers, dialysis staff, and care team
• Serve as liaison between providers, patients, families, and facility staff
• Communicate patient needs and changes in condition promptly
• Maintain accurate documentation per standards and regulations
• Demonstrate professionalism and manage multiple priorities
• Contribute to team collaboration and operational efficiency
• Utilize MS Word and Excel effectively
