Job summary

Ennoble Care is seeking a full-time Patient Care Coordinator in the Oklahoma City area to support care coordination and documentation within an office-based team. You will work closely with clinicians to ensure timely follow-up and smooth transitions across care settings.

The role emphasizes in-office collaboration, strong organizational skills, and professional communication across teams using Microsoft 365 tools and an EMR system.

Qualifications

• Legally authorized to work in the United States.
• Active, unrestricted clinical certification or licensure (MA/CMA, LPN, RN).
• Minimum 1 year of clinical/administrative or care coordination experience.
• Proficient with Microsoft Teams, email, phone and internal systems.

Responsibilities

• Coordinate patient-centered care activities using EMR.
• Support providers with documentation, follow-up and care coordination.
• Act as a primary communication link between providers and internal teams.
• Communicate patient needs and urgent concerns to clinical team promptly.
• Assist with care transitions and referrals as needed.
• Collaborate with Care Coordinators in an office-based setting.
• Maintain timely documentation following workflows and escalation protocols.
• Contribute to team collaboration and office operations.
