Job summary

Ennoble Care seeks a full-time Care Coordinator for an in-office role in downtown Tulsa, Oklahoma. This position supports patient-centered care, documentation, and timely follow-up across care settings within a family-practice-style model.

Working closely with clinicians and care teams, you will ensure continuity of care, accurate records, and proactive communication. A strong emphasis is placed on teamwork, professional conduct, and a healthy work-life balance.

Qualifications

• Legally authorized to work in the United States.
• Active, unrestricted clinical licensure (MA/CMA, LPN, RN).
• Minimum 1 year of experience in a clinical/administrative or care coordination role.

Responsibilities

• Coordinate patient-centered care activities using the EMR system.
• Support care providers with documentation, follow-up, and coordination.
• Serve as a communication bridge between providers and internal teams.
• Assist with care transitions, referrals, and coordination with external partners.
• Maintain timely documentation and support efficient clinical operations.
