Qualifications

• Legally authorized to work in the United States of America.
• Active, unrestricted clinical certification or licensure required, such as:
• Patient Care Technician (PCT)
• Certified Nursing Assistant

Responsibilities

• Provide on-site engagement with dialysis patients to support care plans and education.
• Coordinate patient care using Ennoble Care’s EMR to ensure continuity and timely follow-up.
• Schedule patient visits for providers and support daily clinic workflow based on priorities.
• Collaborate with providers, dialysis staff, and a multidisciplinary team for integrated care.
• Serve as a liaison between providers, patients, families, and facility staff with clear communication.
• Communicate patient needs and changes in condition to the clinical leadership promptly.
• Identify and resolve barriers impacting patient care coordination and experience.
• Maintain accurate, timely documentation per organizational standards and regulations.
• Demonstrate professionalism, empathy, and sound judgment while managing multiple priorities.
• Contribute to team collaboration and facility partnerships with strong organizational skills.
