Job summary

VOALA in Compton, CA is seeking a dedicated Case Manager (Specialist) to provide intensive case management to eligible clients. You will meet participants weekly, coordinate services, and serve as the primary contact for care coordination across a range of human services resources.

The role requires an Associate’s Degree in Social Work or related field (or 2 years of combined education/experience) and at least 1 year of relevant case management work.

Qualifications

• Fingerprint clearance and background check required.
• Associate’s Degree in Social Work or related field or two years’ total education/experience.
• Minimum 1 year of general experience in case management or similar social services role.
• Completion of VOALA Case Management training within 6 months of hire.

Responsibilities

• On-site service provision to clients daily.
• Assess clients and coordinate care plans with services.
• Provide coaching, social support and advocacy to participants.
• Collaborate with team, document progress, and coordinate care.
