Qualifications

• Associate’s Degree in Social Work or related field, or combination of education and experience.
• Minimum 1 year general experience in case management or similar social services.
• Fingerprint clearance and background checks are required.

Responsibilities

• Conducts comprehensive screenings and assessments to develop a case plan.
• Coordinate care and case management with participants and teams.
• Advocate for clients before other agencies and services.
• Provide service linkages and support systems to meet identified needs.
