VOALA is seeking a Case Manager (Specialist) to provide intensive case management services to eligible clients, meeting participants weekly and coordinating services as their primary point of contact. The role involves handling a caseload of individuals in need of diverse human services resources and delivering the full range of available services.

The position emphasizes on-site work, development of action plans, and coordinating care with a service team.

Qualifications

• Must pass fingerprint clearance and background check.
• On-site work is required; report to work on-site daily.
• Minimum 1-year general experience in case management or similar social services role.
• Associate’s Degree in Social Work or related field or combination of education and relevant experience.
• Completion of VOALA Case Management training within 6 months of hire.
• Preferred: Counseling Certification.
• Preferred: 2 years of experience with target population.
• HMIS navigation is a preferred qualification.

Responsabilités

• Conducts comprehensive screenings and assessments to collect information for case plans.
• Provide service linkages and support systems to ensure identified needs are met.
• Coordinate planning with clients to meet short- and long-term goals.
• Coordinate case conferences with residents, consults with service team, monitor progress toward goals.
• Advocacy for participants before agencies and community settings.
• Coordinate care and case management of assigned caseload as part of a team.
