Job summary

Cortica is seeking a Site Care Coordinator to join our multidisciplinary team in San Rafael. You will optimize client and provider scheduling, serve as primary contact for families, and collaborate with on-site leadership to maintain high standards of care.

This on-site role involves managing cancellations, rescheduling, PTO, waitlists, and coordinating with RCM, Scheduling, Enrollment. You will document interactions in Salesforce and support Schools contracts and Value-Based Care clients.

Qualifications

• 4+ years scheduling experience in ABA services or related field.
• 2+ years in client services related role.
• Strong problem-solving skills.

Responsibilities

• Guide patients through care pathways efficiently, monitoring progress and addressing delays/barriers.
• Serve as the point of contact for families, streamlining communication and guiding them through the Cortica journey.
• Keep families informed about wait times for services.
• Create scheduling plans that adhere to best practices, ensuring providers are utilized to full capacity.
• Collaborate with clinical teams to integrate strategies into broader care plans.
• Provide scheduling support for contract scheduling processes, especially for Schools-related contracts and Value-Based Care clients.
• Efficiently handle client cancellations and provider call-outs by arranging alternate coverage or rescheduling.
• Manage PTO requests to avoid disrupting service delivery and forecast gaps.
• Manage waitlists to promptly place clients into available slots.
