Job summary

Providence Willamette Falls Medical Center is seeking an LCSW to perform biopsychosocial/spiritual assessments and coordinate comprehensive care with the Inter/Multidisciplinary Team. The role supports inpatients and outpatients, following NASW ethics and professional standards.

You will collaborate to develop evidence-informed, person-centered care plans, advocating for patients and families while ensuring respect for self-determination and health equity.

Qualifications

Master's Degree in Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the CSWE.
Oregon Clinical Social Worker License or Washington Clinical Independent Social Worker License upon hire.
License in states wherever care is delivered.
2 years relevant Social Work experience. May include CSWA experience.

Responsibilities

Independently provides comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems.
Collaborates with patient and support system to develop person-centered care plans across the care continuum with the IDT.
Works with inpatient or outpatient populations and adheres to NASW ethics and SOP.
