Job summary

Providence is seeking a Licensed Clinical Social Worker (LCSW) to independently provide biopsychosocial/spiritual assessments, diagnoses and therapies. You will work with a multidisciplinary team to develop care plans and advocate for patient-centered care across the continuum.

In this role, you will contribute to reducing health disparities and uphold NASW ethics while collaborating with the IDT. Oregon licensure or Washington license is required, with additional state licenses as care expands.

Qualifications

• Master's Degree in Social Work from a CSWE Accredited School of Social Work.
• Oregon CSW license upon hire or Washington Clinical Independent Social Worker License upon hire.
• License in states wherever care is delivered.
• 2 years relevant Social Work experience.

Responsibilities

• Provide comprehensive biopsychosocial/spiritual assessments, diagnoses and/or treatments.
• Develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the IDT.
• Collaborate with patients and support systems to respect self-determination and reduce health disparities.
