Job summary

Providence Willamette Falls Medical Center is seeking a Licensed Clinical Social Worker (LCSW) for Social Work Services on-call, day shifts. You will independently assess and treat patients, collaborating with the Inter/Multidisciplinary Team to create care plans across the continuum.

You will ensure adherence to NASW ethics and provide person-centered support across inpatient and outpatient settings, with a focus on reducing disparities and supporting vulnerable populations.

Qualifications

• Master's Degree in Social Work from a CSWE Accredited School of Social Work or in candidacy status by CSWE.
• Licensed independent Social Worker in Oregon or Washington upon hire, with license in states where care is delivered.
• Minimum 2 years of relevant Social Work experience.

Responsibilities

• Conduct biopsychosocial/spiritual assessments and document diagnoses and treatment plans.
• Develop and implement evidence-informed person-centered care plans with the IDT.
• Collaborate with inpatient and outpatient teams to coordinate care and referrals.
• Advocate for patient self-determination and reduce health disparities.
