Job summary

Providence Willamette Falls Medical Center is seeking an experienced Licensed Clinical Social Worker (LCSW) for on-call day coverage. You will conduct biopsychosocial/spiritual assessments, diagnoses and interventions, coordinating with the IDT to implement person-centered care plans for both inpatients and outpatients.

You will collaborate with diverse care teams, uphold NASW ethics, and advocate for patients while navigating care across settings.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Oregon Clinical Social Worker License or Washington Clinical Independent Social Worker License upon hire.
• License in states wherever care is delivered.
• Minimum 2 years relevant Social Work experience (CSWA experience may apply).

Responsibilities

• Provide biopsychosocial/spiritual assessment, diagnosis and treatment/interventions.
• Collaborate with the Inter/Multi-Disciplinary Team to develop person-centered care plans across the care continuum.
• Advocate for patients and families and uphold NASW ethics and Standards of Practice.
• Work with inpatient and outpatient populations across the care continuum.
