Job summary

Providence is seeking an LCSW to independently assess, diagnose and treat within an interdisciplinary team in Oregon. You will work with patients and families to create person-centered care plans across the care continuum.

The role emphasizes ethical practice, advocacy, and reducing health disparities, with opportunities to collaborate across departments and care settings in a large health system.

Qualifications

• Master's degree in Social Work from CSWE-accredited program or CSWE candidacy status.
• Oregon Clinical Social Worker License or license in states where care is delivered.
• 2 years of relevant Social Work experience (CSWA experience possible).

Responsibilities

• Independently provide biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy.
• Collaborate with patient and support system to develop evidence-informed care plans across the care continuum.
• Adhere to NASW code of ethics and mission while reducing health disparities.
