Providence Willamette Falls Medical Center in Oregon City is seeking an LCSW to provide comprehensive biopsychosocial assessment, diagnosis and interventions for patients and their families, on-call and day shifts. The licensed social worker will collaborate with the care team to develop evidence-informed, person-centered care plans across the care continuum, upholding NASW ethics and reducing disparities.

Requires master's in social work (CSWE) and Oregon or WA licensure, with at least two

• Master's Degree in Social Work from a CSWE-accredited program.
• Oregon Clinical Social Worker License or Washington Clinical Independent Social Worker License.
• At least 2 years of relevant Social Work experience.

Responsabilidades

• Provide biopsychosocial assessments and diagnoses for patients and their support systems.
• Collaborate with the Inter/Multi-Disciplinary Team to develop person-centered care plans.
• Advocate for patients while maintaining NASW ethics and reducing care disparities.
