Job summary

Providence Willamette Falls Medical Center is seeking a Licensed Clinical Social Worker (LCSW) for on-call day coverage in Social Work Services. The role entails biopsychosocial/spiritual assessment, diagnosis, treatment/interventions, and advocacy for patients and their families.

You will collaborate with the IDT to implement person-centered care plans across the care continuum. The LCSW will serve both inpatient and outpatient populations, upholding NASW ethics and the organization’s mission

Qualifications

• Master's Degree in Social Work from a CSWE Accredited School of Social Work.
• Oregon Clinical Social Worker License or Washington Clinical Independent Social Worker License.
• Licenses in states where care is delivered.

Responsibilities

• Independently provides comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems.
• Collaborates with patients and support systems to develop evidence-informed care plans across the care continuum with the IDT.
• Works with inpatient or outpatient populations and adheres to NASW ethics and mission.
