Providence is seeking a Licensed Clinical Social Worker (LCSW) to independently assess, diagnose and treat patients and support systems using biopsychosocial/spiritual approaches. This role collaborates with the IDT to implement evidence-informed, person-centered care plans across the continuum, while upholding NASW ethics.

Required licensure in Oregon (or Washington) and CSWE-accredited MSW; at least 2 years of relevant social work experience.

Competenze

• Master's Degree in Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the CSWE.
• Oregon Clinical Social Worker License upon hire. Or Washington Clinical Independent Social Worker License upon hire.
• License in states wherever care is delivered.
• 2 years relevant Social Work experience.

• Independently provide biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems.
• Collaborate with patient and support system to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the IDT.
